New IVF technology at Genea Fertility sees spike in live birth rate and gives Perth parents fresh hope

Fifteen-month-old Zara plays with a wooden tea set, her plate piled with faux cupcakes and lamingtons. Proudly watching on, her parents Elle and Michel Buonaiuto can’t quite believe she came from the same bundle of cells they first viewed on their phone screens. The couple tried to conceive their first child for a year before turning to assisted reproductive services, where they learnt Mrs Buonaiuto’s existing endometriosis and blocked fallopian tubes meant the chance of falling pregnant naturally was slim to none. The IVF process began with an egg stimulation and retrieval, which produced four good-quality embryos. One became the bubbly, confident Zara, and, after a second transfer earlier this year, Mrs Buonaiuto is now 21 weeks pregnant with a son. To build their family from a single retrieval is beyond their wildest dreams. “I know some people struggle for years, I am under no illusion that we are some of the lucky ones who have been successful,” Mrs Buonaiuto says. “We entered this journey expecting it would take a long time. We never expected it would be just one cycle and we could get our kids from that.” The science that made it possible Mrs Buonaiuto’s two pregnancies may not have eventuated without a major development in assisted reproductive technology. Genea Hollywood Fertility recently announced a milestone 12.2 per cent increase in live births thanks to its cutting-edge Geri incubator, which aims to more closely mirror the conditions found in a mother’s womb. Not only does the incubator contain an advanced fluid for the embryos to develop in but in-built cameras take photos of the cells every five minutes so scientists can review the embryos without disturbing them. The technology is only available in WA at Genea Hollywood’s clinic, where Mrs Buonaiuto was treated by Dr Michael Allen. While many in the field will choose to focus on one area, Dr Allen specialises in a broad spectrum of women’s health from fertility to obstetrics. This continuity of care allows him to marvel at the sheer miracle of creating a human life. “To see embryos going back into a patient at 140-odd cells and then see billions of cells come out nine months down the track, and watch that little person take their first breath, it is incredible,” Dr Allen says. He was there for the arrival of Zara and plans to deliver her baby brother in February. “It’s been been a lot more personal going through the journey with Dr Allen and having this one-stop-shop between the fertility stages and birthing process,” she says. “He knows our whole story and where we have been.” A high-tech vantage point It’s not just the scientists who get to view the embryos developing in those crucial first five days via Geri’s cameras. Genea’s patients have 24/7 access to a world-first app, Grow, which allows hopeful parents to watch time-lapse videos of the replicating cells that may one day become their child. “I have a picture of Zara as an embryo, which is pretty crazy to think about,” Mrs Buonaiuto says. “To see the science and watch it all happen from so early on was amazing. In the first five days, we could watch the fertilisation process of those embryos through our phone.” She adds the app took away a lot of the confusion and uncertainty often experienced in the IVF process. “We were very much a part of the process; we weren’t left in the dark at all.” First time’s a charm The Buonaiutos are a heart-warming example of the “One Cycle, One Family” mission among the doctors at Genea Hollywood. Their aim is to help patients complete their family in just one stimulated cycle, which Dr Allen explains is often the most taxing part of the IVF process. “It’s the egg collection, with the hormone injections, day surgery procedure and cost,” he says. “In contrast, the subsequent use of frozen embryos is far simpler, potentially drug free and much less expensive ... If we can reduce the number of stimulation cycles for patients, it presents a significant physical, cost and emotional benefit.” Achieving a higher quantity of good quality embryos through the Geri incubators has proven to not only minimise the need for further cycles but also result in a live birth — the ultimate goal of those who embark on the reproductive rollercoaster. For the couple, the possibility of having further babies from their single retrieval is an exciting prospect. “Ending up with four high-grade embryos was a massive relief,” she says. “We haven’t decided how many children we are going to have so it’s good to know we still have two left when we need them.” Why is the increase in the live birth rate so significant? The Geri incubator was first introduced in 2016, but it takes several years to collate enough data to prove the impact on fertility outcomes. While Genea fertility specialist Dr Mike Allen had observed first-hand an increase in his IVF patients having babies, he did not expect the live birth percentage rise to be so high. “All of our jaws hit the floor when we got that information,” Dr Allen says. “All our patients have use of this incubator without paying anything extra, we really feel they all deserve to have access to this technology.” Dr Allen explains the Geri incubators provide a better selection of embryos to transfer into patients. “The original incubators would have to be open and closed by embryologists,” he says. “The embryos would be taken out and put under a microscope and exposed to things like light and humidity. That can have a negative impact on the embryo. A naturally-conceived embryo inside a patient is obviously protected from those things. “In the incubators we have now, the embryo doesn’t need to be disturbed and it is photographed over the course of its first five days. That is a tool for the embryologist in order to select which are best, so the embryos going into the patients have the highest chance of resulting in an ongoing pregnancy. That’s where the 12.2 per cent increase in the live birth rate is coming from.”
